Observation

Ground cover did not measure 9 inches of pea gravel in all areas. One gate observed to have gap more then 3.5 inches. Technical assistance was provided on fall zones and removal of broken equipment, clear emergency exit pathways. Outdoor emergency exit was observed locked during onsite visit.
